May 2012 - Durong
Workplace Health and Safety Queensland is investigating a fatal incident that occurred on Tuesday 8 May 2012 at Durong.
A tip truck driver died when he was working alone in an isolated area, preparing his tipper for the day's operations. He was crushed between the cabin of his truck and a tree.

Risks associated with lifting formwork
The purpose of this safety alert is to highlight the risks associated with lifting formwork and the need to make sure the lifting method is safe.
A construction worker died when a wall form dropped while being lifted with a two-legged sling. It would appear that the two vertical timber posts that the lifting lugs were attached to failed.

November 2011 - Brisbane
Workplace Health and Safety Queensland is investigating a fatal incident that occurred on Monday 31 October 2011 at a construction site within the University of Queensland, St Lucia Campus, Brisbane.
A worker was killed while a formwork shutter was being raised to about one metre above the ground by crane. Lengths of timber were being used as lifting points for the shutter when one of the pieces of timber broke. The formwork shutter dropped to the ground and fell over, striking the worker.

November 2011 - Blackall
Workplace Health and Safety Queensland is investigating a fatal incident that occurred on Tuesday 15 November 2011 at a property in Blackall.
A farmer was killed when he fell off and then was struck by the water truck he was riding on. The farmer was standing on the step plate and holding onto the bulk head of the truck while carrying out controlled back burning on the property.

March 2012 - Brisbane, Mackay & Toowoomba.
Workplace Health and Safety Queensland is investigating three fatal incidents that occurred last week in Queensland.
-
A worker was electrocuted while working on a billboard in the western Brisbane suburb of Sherwood.
-
A young worker was electrocuted on a construction site at Clermont, 250 km south west of Mackay.
-
A worker was killed during the erection of an electrical transmission tower at Jandowae, north-west of Toowoomba. A section of the tower fell while being lifted by a crane.

A worker was killed while welding on a waste oil tank when an explosion occurred.

March 2012 - Brisbane
Workplace Health and Safety Queensland is investigating a fatal incident that occurred on Tuesday 20 March 2012 at the Port of Brisbane.
A labour hire forklift operator was killed when he was crushed by the digging/drilling rig attachment of a toppling excavator. It is not yet known what caused the excavator to become unstable and topple.
